What the service offers Health visitors support families with pre school children in a variety of different settings including people’s homes, health centres, community premises and clinics. The service aims to promote well being and maximize health gains.Health visitors deliver a core surveillance programme to children including screening children for health problems.The service promotes good health and provide health and social care advice and act as a resource for families and colleagues. Health visiting is a universal service that is well placed to identify and respond to child protection concerns when they arise and work in partnership with other agencies to safeguard and protect children.
Who is the service for? The whole of Monmouthshire. The service is predominantly, although no exclusively for children from birth-4 years and their families.
What is the procedure for referrals? Every child under 5 years will have a named health visitor. Health visitors are attached to health centres of GPs. They can be contacted by telephone at health centres or via other members of the Primary Health Care Team such as a GP. A list of where health visitors can be contacted is available from Risca Health Centre.
Opening hours? Monday to Friday 9am-5pm.
